Cobalamin deficiency
- 1000 mcg/day ORALLY, IM route is preferred; 30 to 50 mcg IM daily for 2 or more wk; then 100 mcg monthly to sustain remission

Instruct patients using the intranasal form to report nasal congestion or rhinitis, as this may affect drug absorption.
This drug may cause transient diarrhea or headache.
Patient should take extended-release tablets with a meal and not within 2 h of other medications.
Advise patient to take the intranasal gel at least 1 h before or 1 h after ingesting hot foods or liquids.
Tell patient to leave sublingual tablet, lozenge, or spray in mouth or under tongue for 30 seconds before swallowing.
Patient should not drink alcohol while taking this drug.

- prime Nascobal(R) nasal spray actuator initially and before each use; administer at least 1 hr before or 1 hr after ingestion of hot foods or liquids
- prime CaloMist(TM) nasal spray pump prior to the first use; re-prime if the pump is not used for 5 or more days; separate administration with other intranasal medication by several hours

- (time-released) preferably take with a meal
- (time-released) do not take within 2 hr of medications
- (sublingual) place tablet under tongue for 30 sec before swallowing
